Before Your Visit
At Fraser Street Medical, we want your visit to be smooth, efficient, and safe. Here’s what you can do to prepare — whether your appointment is in person or by phone / telemedicine.
What to Bring
To help your healthcare provider give you the best care possible, please bring:
- Your BC Services Card (CareCard) or photo ID

An up-to-date list of your medications, including vitamins and supplements
- Any recent lab or imaging results (if done outside FSM)
- Your pharmacy name and phone number

Any forms or letters that need to be filled out (insurance, work, school, etc.)
Before a Telemedicine Appointment
If you are seeing your provider by phone or video, please:- Keep your phone nearby and ensure it’s charged
- Find a quiet space with good reception or Wi-Fi
- Have a list of your current symptoms and questions ready
- Make sure you are in British Columbia during the appointment (MSP coverage applies only to BC residents physically in the province)
When You Arrive In Person
- Please arrive 10–15 minutes early to check in.
- Bring your CareCard and masks are optional but encouraged for patients with symptoms.
- If you’re a new patient, fill out the New Patient Intake Form before your visit.

After Your Appointment
- Lab and test results are usually reviewed within 1–2 weeks.
- Your provider will contact you if results need follow-up.
- If you haven’t heard from us after two weeks, please contact the office.
