What is an AI Scribe?
An AI scribe is an advanced tool that securely transcribes and organizes medical notes during your visit. This improves documentation, streamlines care, and allows your clinician to focus more on you.

Is My Information Private and Secure?
Absolutely. Our AI scribes comply with PIPEDA, ensuring your health information is encrypted, confidential, and only accessible by authorized staff. Data is not stored beyond what is necessary for documentation.

Do I Need to Sign a Consent Form?
Yes, but only once. FSM is gradually sending consent forms via email, and your clinician will always inform you when an AI scribe is in use.

What Do I Need to Do?
Nothing! Your visit remains the same. If you have any questions, feel free to ask your clinician.

Can I Opt Out?
Yes. If you prefer not to use an AI scribe, let your clinician know, and we will document your visit manually.

Is It Accurate?
Yes. AI scribes generate initial notes, but your clinician always reviews and proofreads them to ensure accuracy before finalizing.

Nurse Practitioners are primary care clinicians that provide comprehensive care including the diagnosis and management of disease/illness, prescribing medications, ordering/interpreting laboratory/diagnostic tests, and initiating referrals to specialists. Nurse Practitioners work as independent primary care providers and develop a patient panel similar to a Family Physician. At Fraser Street Medical Clinic, Nurse Practitioners work in collaboration with other providers in the clinic by cross covering and ensuring optimum patient care for all who attend the clinic.

The CDM Nurse works with family physicians to support management of patients with chronic illness. In addition to the role, CDM Nurse carries out other clinical tasks such as baby wellness, immunizations, taking vital signs, administer medication, patient education, and many more.

We try to accommodate all patients who have an urgent issue. We have appointments that you can book in advance, but we also save same-day appointments available for when your medical issue is of a more urgent nature.

Fraser Street Medical has walk in appointments only for our own registered patients. Our regular hours are Monday-Friday from 8:15 AM-5:00 PM, Saturday from 8:00 AM-12:00 PM (closing time varies on Saturdays) to help with same day urgent visits for our patients.

We try our best to accommodate walk in patients, and will try to fit you in with your own doctor, depending on availability. However, wait times can fluctuate depending on how busy we are; our staff can usually give you a general idea of how long the wait may be.

At Fraser Street Medical (FSM), we are committed to providing high-quality, patient-centered care. To enhance documentation, improve efficiency, and allow clinicians to focus more on patients, FSM will implement AI scribes as part of our medical practice.

1. Purpose

The AI scribe is an advanced tool that securely transcribes and organizes medical notes during patient visits. This technology supports accurate documentation, streamlines workflows, and enhances the overall patient experience.

2. Implementation & Use

  • AI scribes will be used during medical visits to assist clinicians with real-time documentation.
  • Clinicians will review and proofread all AI-generated notes before finalizing records.
  • Patients will be informed when an AI scribe is in use and can request manual documentation if preferred.

3. Privacy & Security

  • AI scribes at FSM comply with PIPEDA (Personal Information Protection and Electronic Documents Act).
  • All data is encrypted, confidential, and only accessible by authorized personnel.
  • Patient information will not be stored beyond what is necessary for medical documentation.

4. Patient Consent

  • Patients must provide one-time consent before an AI scribe is used in their care.
  • FSM will gradually introduce consent forms via email and in-person visits.
  • Patients may opt out at any time by informing their clinician.

5. Benefits to Patients

  • More clinician focus – reduced screen time and better patient interaction.
  • More accurate records – complete and well-organized documentation.

6. Patient Rights & Opt-Out Policy

  • Patients have the right to decline AI scribe use without any impact on their quality of care.
  • Alternative manual documentation will be provided upon request.

7. Questions & Support

Patients are encouraged to speak with their clinician if they have any concerns or require further information about AI scribes. FSM remains dedicated to innovation, security, and excellence in patient care.

  • Same-day fit-in appointments are available for urgent medical concerns.
    Walk-in appointments are accepted based on availability (for registered patients only).
  • Urgent concerns include:
  • Persistent respiratory symptoms (cough/cold lasting more than a week)
  • Infections (e.g., UTIs)
  • Eye concerns (red eye, blurry vision)
  • Allergic reactions

1) Late Appointments – If you are more than 10 minutes late, you may be asked to re-book your appointment.

2) Missed appointment – There will be a $35 charge if you do not provide at least 24 hours cancellation notice.

Our No Show Policy and Late Visit Policy will be enforced at the discretion of the physician. You may be called in to review this policy in person with your physician if there is a pattern of no shows or late appointments. Fees may apply for NO SHOWS.
