Fraser Street Medical Patient-Clinician Agreement
FSM and our family physicians believes this compact appropriately describes the relationship between a FP and their patients.
Agreement:
As your family doctor, I agree to:
- Provide you with the best care possible.
- Coordinate specialty care as needed.
- Offer timely access to care.
- Maintain an ongoing health record.
- Keep you updated on clinic changes.
- Communicate openly with you to address your health needs.
As my patient, I ask that you:
- Seek care from me and my team whenever possible.
- Identify me as your family doctor when visiting emergency facilities or other providers.
- Communicate openly to ensure we can address your health needs.
- Update your patient information (phone number, address, etc.) with the clinic to maintain accurate records.
Consent for AI Scribe & Digital Communication:
- I consent to the use of AI scribe technology during consultations in compliance with Canadian privacy laws (PIPEDA).
- I consent to receiving emails and text messages for appointment reminders and relevant clinic updates.
No-Show & Late Arrival Policy:
- Cancellations: Notify at least 24 hours in advance.
- Punctuality: Arrive at least 5 minutes early.
- Fees: A $35 fee applies for missed appointments without proper cancellation.
