Also, to give you a chance to learn about your chronic disease, and get help from Fraser Street Medical’s team, please book a joint clinician/pharmacist in-office medication review. You, your clinician, and pharmacist will review your drug therapy, and help ensure you get the best chronic disease care. If you are a regular patient of the clinic and are on at least 5 medications/prescriptions, you can call our office to book a medication review appointment.
