INSTRUCTIONS:
You don’t need a password.
You just your personal information such as your First name,
Last name, Care card number and Birthdate, to make an appointment!
You can book 1 appointment, ONLY with your own health care provider.
PLEASE PAY ATTENTION TO THE FOLLOWING
1.Telemedicine Visits are remote visits, which means that you can see your Personal Health Care provider without leaving your home.
You should NOT BE COMING TO THE OFFICE for telemedicine visits.
2. If you require an IN-OFFICE VISIT, please call our office at 604-322-3366 and speak to one of our receptionists.
3. You should have some information ready for the time you book, such as:
A. The fax number of your Pharmacy
B. An updated email address
C. You should already have the following consent form filled out prior to booking an appointment online:

The only way you can book is Online, as we need to screen carefully for Covid and due to social distancing and cleaning protocols.
If you were Covid positive or have symptoms or have been travelling, do not come to the office. If you do come to the office, you will get a phone call from the medical director regarding this safety issue.
We ask that all patients give us the exact details of their inpatient booking requests as we will only see essential visits to ensure safety of our staff and patients.
Medical masks are required for entry to the office.
