OpTIC TEAM BASED CARE STUDY PATIENT SURVEYS
As a patient at Fraser Street Medical, we would like you to know that we are constantly working to improve our services, team, and care for you.
Fraser Street Medical is participating in OpTIC-PHC, a CIHR-funded research study by UBC on how professionals in a primary care team
work together. Your experience will help us strengthen the care we deliver to you.
We are inviting patients to share their experience through a brief interview:
• Who can participate: Any patient currently receiving care at Fraser Street Medical (or family caregivers of patients)
• What: One-on-one interview about your experience at the clinic
• Duration: 30–40 minutes
• Where / Format: A UBC researcher will chat with you in-person, by phone, or via Zoom (at your convenience)
• Honorarium: $40 gift card
• Confidential and voluntary (your medical care will not be affected)
