 How to Book:

• No password required

• You must provide the following:

  • First Name

  • Last Name

  • Personal Health Number (Care Card)

  • Date of Birth

• You may book only one appointment at a time with your own health care provider.

 Please Read Carefully:

1. Telemedicine Visits are virtual only.
   Do not come to the office for these appointments.
